Please note all centers will resume normal operations on Wednesday, September 9, 2015. Due to the closures we expect heavier traffic in our centers during that week, so please plan accordingly and consider using online services to avoid a visit altogether."

Send Your Secure ID Documents By Fax or Mail

In addition to our online option, customers can send their missing "Proof of Residence" and "Proof of Social Security Number" documents by fax or mail.

Sending Documents By Fax

A customer will need to log into their DDS online account, select "SecureID: Submit proof of residency or social security" from the account home page and use the Fax/Mail option to print a cover letter for sending your documentation by mail.

Fax the cover page barcode document and copies of your Secure ID documents to (770) 918-6279.

Sending Documents By Mail
